Six Steps to Success End of Life Care Programme

The Six Steps to Success End of Life Care Programme was developed in the North West of England to enhance the knowledge and skills of care staff working in care homes

Based on The Route to Success: A Guide to Improving End of Life Care in Care Homes (2010), the programme aims to ensure all residents receive high-quality, person-centred care from confident and competent staff. It is regularly updated to reflect current best practice in palliative and end of life care

Each participating care home nominates one or more representatives who lead on implementing the Six Steps approach and driving organisational change

The programme includes six workshops designed to empower representatives to embed the Six Steps to Success within their organisation

  • Stepping On Beginning the programme
  • Step 1 Discussions as the end of life approaches
  • Step 2 Assessment, care planning and review
  • Step 3 Coordination of care
  • Step 4 Delivery of high quality care in care homes
  • Step 5 Care in the last days of life
  • Step 6 Care after death
  • Stepping Off Concluding the programme
