This study day is suitable for All Health care Professionals working in Primary, Community, Acute, Hospice, Mental Health and Social Care settings who are involved in supporting people with future care planning
Build the confidence to have a meaningful conversation about future care
This interactive session will equip you with the knowledge and practical skills to support patients in exploring, recording and sharing their wishes, preferences and priorities for future care
- Think About It – understanding Advance Care Planning, what it is and what it isn’t, explore Mental Capacity and ‘Best Interests’ decision making
- Talk About It – develop confidence in starting and managing sensitive conversations, exploring patients’ values, wishes and preferences
- Record It – understanding the key elements of Advance Care Planning and how to accurately document wishes, preferences and treatment decisions
- Share It – how to ensure that relevant information is shared effectively across services and applying your learning to improve person centred care in your practice
