An opportunity to explore how to find out the patient’s views, preferences and wishes regarding their future care and feel confident and competent to assist in documentation

• Think About It – understanding Advance Care Planning, what it is and what it isn’t, Mental Capacity and ‘Best Interests’ Decisions
• Talk About It – opening and maintaining a conversation about future wants and preferences
• Record It – what elements of Advance Care Planning are there, how to document wishes, preferences and refusal of treatment
• Share It – how to ensure that relevant information is shared, cross boundary information sharing, how to take this back into practise
