HEA4219 Transforming End of Life Trajectories

A 20 credit interactive, clinically based module is aimed at frontline clinicians, of all professions, providing core or specialist level palliative care. It will be of particular interest to District Nurses and General Practitioners, hospital ward nurses and trainee/SASS doctors, hospice nurses and doctors, allied health professionals whose work encompasses interaction with those approaching the end of life.

The module includes:

• trajectories of end of life in all disease groups
• recognition of those approaching the end of life
• assessment and management of common symptoms
• co-ordination of care
• future care planning to include anticipatory clinical management planning
• available resources and services for patients and families
• dealing with, and communicating, uncertainty in the
deteriorating patient
• managing changing multiple co-morbidities in association with a terminal illness

The module has been designed by Queenscourt Hospice
in partnership with Edge Hill University. The programme
will be delivered in a blended fashion online and at
Queenscourt.
