Carer Check In
This session is for Health Care Professionals
The Carer Check-In Project was developed in response to several needs:
Consistent feedback from Carers, staff and educators that multiple pressures on health and social care staff were leading to little or no engagement and communication with unpaid Carers of patients approaching the end of their lives. This was made worse by the enduring impact of the COVID pandemic on staff and carers
Requests from local senior clinicians and carer’s groups of the need for something with small, manageable steps, that re-initiates the connection between staff and carers
Something which does not feel to staff as though it is adding something new to their already overstretched workload.
The National and North West Coast Network Delivery Plan Priority to support the assessment, well-being and resilience of carers
